FAMILY MEDICINE CENTER

1)Patients will only be seen by Appointment only. We are recommending all patients call before entering our office if you do not have a scheduled appointment. 

2) A mandatory temperature screening as well as a Covid-19 triage screen will need to be completed for every patient entering the building.

3) Visitor Restrictions are still in effect. Please do not bring additional family members to appointments unless assistance is needed.
