As part of a PCMH, you will receive more coordinated, comprehensive, personalized health care that supports you and strengthens your relationship with your doctor. Good medical care means more than merely treating episodes of illness. Periodic health survey for the protection and early detection of illness is equally important. This consists of a review of one's routine tests and consultations with a physician. All patients should be seen annually for preventive care.
