We are committed to enhancing the delivery of the highest quality primary care to residents of Southern Ocean County through care coordination, efficiency, and practice transformation activities. Acting as your medical home, our goal is to provide holistic healthcare and assistance in navigating our complex healthcare system.

As a Patient Centered Medical Home (PCMH), we continue to make changes to better our practice and meet your healthcare needs. This means our medical practice successfully meets new national standards to provide you with responsive and higher quality health care. As part of this commitment, we have a dedicated care management team at our practice. They will help make sure you get the care you need, at the right time and place. As part of a PCMH, you will receive more coordinated, comprehensive, personalized health care that supports you and strengthens your relationship with your doctor. Good medical care means more than merely treating episodes of illness. Periodic health survey for the protection and early detection of illness is equally important. This consists of a review of one's routine tests and consultations with a physician. All patients should be seen annually for preventive care.

Family Medicine Center has been recognized by the National Committee for Quality Assurance as a Level 3 practice on achievement of recognition for systematic use of patient-centered, coordinated care management processes. OUR TEST RESULT NOTIFICATION POLICY

  • Please allow 7-10 business days to receive test results.

  • Please call our office if you have not heard from us after 14 days.

  • If a STAT test is ordered you will be notified of those results in the same day.

It is up to you and your physician to determine how you will be notified of results. Options are to be notified by phone, patient portal, or email. Discuss your preference with your doctor!

Please contact our office if you receive notification from the radiology facility that additional or follow-up testing is recommended. You will need a new referral or prescription for additional studies, and may need an appointment to discuss further.

Family Medicine Center has implemented a "Referral Tracking" policy to help avoid any delays in test results being received and reported to better serve you! With new technology and electronic medial records, we want to be sure nothing gets overlooked. We strongly believe in "Patient Engagement" along with following your doctor's recommended "Preventative Measures" is very important in managing your health!
